Transcranial doppler flow velocity in iron deficient Nigerian children with Sickle Cell Disease

Summary

BACKGROUND: Iron deficiency anemia is common in children with sickle cell anemia living in sub-Saharan Africa and is an independent risk factor for pediatric strokes in children without sickle cell anemia. Approximately 11% of the children with sickle cell anemia will have strokes, and the prevalence of iron deficiency anemia in Nigerian children with SCA is very high, approximately 50%.

GAP: The study will determine the relationship between the presence and treatment of iron deficiency anemia on TCD measurements in children with sickle cell anemia.

HYPOTHESIS: Primary hypothesis: Iron deficient anemia is associated with an increased TCD measurement when compared to children who are iron replete.
Secondary hypothesis: For those children with TCD measurement less than 200 cm/sec nd iron deficient, iron supplementation will decrease TCD measurement.

METHODS: Cross-sectional prospective cohort study with a single arm treatment for children with sickle cell anemia that are iron deficient. Patients 5 to 12 years of age with hemoglobin SS or SB0confirmed by hemoglobin electrophoresis who are in steady state and have a ferritin level <30 µg/L, with confirmatory CBC showing anemia suggestive of iron deficiency.

RESULTS: Pending.

IMPACT: After completion of this study, we will better understand the association between iron status and TCD measurements among children with SCD in a low income country with high prevalence of iron deficiency anemia.
